Clinton Co. Health District releases food inspections

The following information is obtained from the Clinton County Health District and is compiled from inspection reports.

Violations are either critical or non-critical. Critical violations are more likely to contribute to food contamination, illness or an environmental health hazard, including inadequate cooking of food items or poor personal hygiene.

DBA Subway (inside Walmart), 2825 St. Rte 73 S., Wilmington

  • Large build up of debris along side the ice machine chute (customer service area)
  • Found one nested lexan container with sticker residue
  • A manger trained food safety individual is needed for the facility
  • Bin containing pass-thrus still in need of a thorough wash (PIC is washing today)
  • Continue to clean top of interior of middle sink of three compartment. Small amount of debris present
  • Fan guard in walk-in cooler needs dusted

Taco Bell #32829, 1701 Rombach Ave., Wilmington

  • There was a knife stored in knife holder with food product on the blade
  • The counter/shelf where the taco’s/ burritos/food are had a build up of food debris
  • No cleaning schedule posted
  • Sugar packets stored under hand sink in drive-thru. Nothing allowed to be stored underneath due to possible contamination
  • Manager with beard was not wearing a beard restraint
  • Vegetable prep sink was dirty and had a tray in it to be washed. Prep sink must only be used to wash/prep vegetables
  • Nozzles on ice machine are dirty with mold/mildew. ICEE is responsible for cleaning
  • Broken countertop in drive-thru
  • Seal is broken on cooler
  • All equipment must be properly cleaned

Streber’s Market, 299 South St., New Vienna

  • Received a complaint regarding outdated lunchables and milk
  • 28 lunchables, one package of bologna and several packages of cheese out of date
  • Must remove products immediately and use proper labeling.
  • Chocolate cake found without proper labeling

The Red zone Neighborhood Grill, 762 W. Main St., Blanchester

  • Stove and Deep-fryer has been cleaned and knew knobs on oven/stove
  • Hot water heater to be replaced
  • Everything fixed from precious inspection
  • Ready to open

Bob Evans LLC, 1075 East Side Drive, Wilmington

  • Found home fries in the Delfield reach in at 45 degree Fahrenheit and batter in small undercounter reach in at 55 degree Fahrenheit.
  • Found previously cooked corn and open bag of spinach without a date mark, must discard on 7th day
  • Found a large amount of condensate at the bottom of the reach in cooler containing batter
  • Found the sides of cookline equipment in need of cleaning
  • Found a large amount of water on the floor near the ice machine, no floor drain observed
  • Found two items not holding 41 degree Fahrenheit or below and two items not date marked for discard in seven days
  • Observed good handwashing by both cooks during inspection

Comb’s Baker, 1221 Wayne Rd., Wilmington

  • Followed up so that walk in freezer could be inspected
  • Did not find violations in walk-in freezer area
  • Wash machine still in need of cleaning
  • Floors behind the aquafina cooler need cleaning
  • Label food in dispensers with common name
  • Correct remaining items

Denver Elementary, 291 Lorish Ave., Wilmington

  • Walk-in cooler still down, using pass- thru cooler and milk cooler
  • Facility is supposed to be getting a new walk-in cooler that will set outside of building in 6-8 weeks. Plans for new walk-in must be submitted as soon as possible
  • Will return in two weeks to check temperature of foods

Wilmington Nursing and Rehabilitation, 75 Hale St., Wilmington

  • Reach-in cooler still not working
  • Walk-in cooler has cart with drinks and portioned food that can be rolled out when preparing lunches/dinners
  • All plumbing corrections must be done by licensed plumber under permit due to this being a commercial facility

Hampton Inn, 201 Holiday Drive, Wilmington

  • Found pats of butter at 50 degree Fahrenheit on customer service counter
  • Employee in the kitchen had hair pulled back but no net, employee corrected before inspector commented
  • Trash can is needed near the handwash sink in kitchen for convenient discard of paper towels (may use had or visor as well)
  • No covered trash can in the restroom
  • Cabinet with the grease trap and cabinet with the carbonator need wiped of spillage
  • Ensure TCS foods are held at 41 degree Fahrenheit or lower to control bacterial growth

Holiday Inn, 155 Holiday Drive, Wilmington

  • No manager trained in food safety certificate found (PIC at time of inspection has food safety training)
  • Found handwash sink blocked with pop, moved to allow access
  • Found hot hold of precooked sausage patties at 114 degree Fahrenheit, items reheated and replaced
  • Found employee preparing food with unrestrained. ensure any employee in kitchen wet hair restraint
  • Observed a water stained ceiling tile in the storage room above boxes of sweetener, no active leak observed

Wilmington Dairy Queen Chambers, 1093 Rombach Ave., Wilmington

  • There were gnats flying around the soda boxes, all facilities must be free from insects/pests
  • Both male employees had facial hair with no restraints
  • Freezer door has lots of frost, probably due to broken seal
  • Blizzard blending machine is dirty, wall of walk in dirty, broken floor tiles in kitchen
  • Currently, not breaking down soft serve machines daily and sanitizing them. All food contact surfaces must be properly cleaned and sanitized

McDonald’s, 201 W. Main St., Blanchester

  • Received a complaint regarding “black” on an egg and cheese biscuit sandwich
  • Manager on duty said it was from the grill, checked temperatures and date marks, everything was properly date marked and all temperatures were good
  • When inspector walked in the kitchen. employee on grill was eating. All employees must refrain from eating while cooking/prepping food
  • Employee on grill had no beard net, employee on drive thru window had no hair net/hat. All employees need hair restraints

East Clinton High School, 175 Larrick Road, Wilmington

  • Inspection is a standard
  • Handwash sinks (with sensor faucets) are not reaching 100 degree Fahrenheit, highest temp. reached was 93 degree Fahrenheit
  • Adjust so water reaches 100 more quickly
  • Water at three comp sink takes 10-15 minutes to heat to 110 degree Fahrenheit, adjust water heater or valves
  • Found the dish machine only dispensing chlorine at 10 ppm – should be 50 -100 ppm for sanitizing, have machine calibrate. Should not be used until re-calibrated
  • Found light in walk in freezer burnt out and door damaged; this door does not open fully, PIC states new door is on order
  • No quat ammonia test strips located during inspection; must order
  • Hot hold cabinets working well during inspection

Sabina Elementary School, 246 W Washington St., Sabina

  • No stem food thermometer available to verify holding, cooking, cooling temperatures
  • Found gnats in the dish room; Told PIC to scrub drains with brush, rinse with very hot water and cover in ice
  • Found steamer not working- sensor does not function correctly
  • Replace silicone on walk in cooler and freezer (replace with silicone that can stand cold temperatures)

New Vienna Elementary, 301E. Church St., New Vienna

  • Silicone on coolers are peeling; resilicone with temperature resistance material
  • Metal drain board gathers water (dirty side) adjust so water drains
  • Metal run ( on wall near dish machine) is not flush with drain board
  • Drain board has a space between the wall
  • Knob missing on hood for selector
  • Must repair all items by next standard inspection
  • Thermometers in working order

Denver Elementary, 291 Lorish Ave., Wilmington

  • Walk in cooler unit not working; unit needs repaired, inside walls need painted/coated replaced
  • Facility has one/two door unit being used to keep food cold/ attached to warmer
  • Employees are prepping food at last minute to try to keep foods cold
  • Additional cooling units need to be provided to deal with all food prep & holding
  • By overloading cooling unit it drives up temperatures of foods and makes unit not work properly

Wilmington Nursing and Rehabilitation., 75 Hale St. Wilmington

  • Received complaint regarding hair restraints, no glove wearing, serving of raw food, plumbing issue with mop sink, garbage disposal and general maintenance/cleanliness
  • There were no paper towels available at the hand washing sink and employees are dumping ice into the hand washing sink
  • Chicken salad at 57 degree Fahrenheit, cottage cheese at 51 degree Fahrenheit and cream cheese at 59 degree Fahrenheit
  • All TCS foods must be kept cold at 41 degree Fahrenheit
  • Hard boiled eggs not labeled or dated. All foods but be properly labeled and dated for use with seven days or they must be discarded
