(Antigo, WI)-The Community Health Foundation is accepting funding applications.
The Community Health Foundation (CHF) is a nonprofit organization that was founded in 1979 by Aspirus Langlade Hospital. “The CHF Mission is to identify needs and award grants to provide for the health care needs of the community through financial and educational support” said Steve Brettingen, Foundation President. “We encourage community organizations that meet our eligibility guidelines to consider submitting a grant application for review.”
Eligibility Requirements Include:
• Demonstrate not-for-profit and tax-exempt status
• Address a local community need as identified by the Community Health Needs Assessment
• Include a plan that addresses the specific use of the funds
• Demonstrate likelihood of beneficial impact for need identified
• Demonstrate ability to provide consistent fiscal and budgetary performance
• Identify other funding sources being utilized to address the need
Recent projects funded by the Community Health Foundation included Antigo Music Association, City of Antigo, Weekend Backpack Program, Boys & Girls Club of the Northwoods, Antigo High School Blue Line Club, Antigo Gridiron Club, Langlade County Area Diaper Bank, and Antigo Area Youth Hockey Association.
Requests for application materials may be found online at aspirus.org/the-community-health-foundation and will be reviewed by the Community Health Foundation quarterly.
The Community Health Foundation is incorporated as a 501(c)(3) not-for-profit corporation and is governed by a volunteer board of directors.
