Botanica Luz De Arcangeles, 739 W. Cypress St. Suite 12, Kennett Square, November 9. Fail. The electrocuter insect control device located in the kitchen area is not designed to retain the insect in the device. Must remove them immediately. Use traps that use light and glue boards. The handwash sink located in the kitchen did not have water at a temperature of at least 85°F at beginning of inspection. Heater was turned up and water was 107F. Ensure all staff know how to turn water temperature up if needed. Pickles, relish and mayo products must be refrigerated after opening. Several open contains found on shelves. All were discarded. Mops must be hung on the wall to dry near the mop sink. Mop water must be dumped in big sink near the back kitchen door. A table top deep fryer was being used to make empanadas in the store. Cannot use this equipment; have to hood or fire system. Remove the deep fryer from the building. The residential air fryer must also be removed.

Prepackaged food is not labeled properly with the ingredient statement and/or nutritional facts for bagels, cinna rolls, and mighty muffins. Jim corrected on site and provided a placard to be posted at breakfast island. Paper towel dispenser empty at the handwash sink in far back kitchen. Kenny corrected on site. The following equipment not observed in good repair: True 2 door freezer in back of kitchen by three bay sink is not operable. Repair, replace, or remove.

Dollar General 16385/W. Fallowfield, 994 Gap Newport Pike, Cochranville, November 9. Pass. Replace the missing drain plug of the dumpster. Defrost the novelty ice cream freezer and maintain in clean condition. Clean the exhaust vent covers in both toilet rooms. Water leak observed at the supply line by the UV light. Make all necessary repairs to stop the leaking.

Duck Donuts, 1353 Wilmington Pike, West Chester, November 9. Fail. Clean the following: exterior of ice machine; steel shelf containing icing coloring/flavoring bottles. In in icing prep area, an open bottle of lime juice was found stored at room temperature although label reads ‘refrigerate after opening.’ Once bottle is opened, lime juice must be stored in refrigerator at 41°F. Note: Lime juice discarded. Clean the floors, floor drains and air gaps throughout the facility. Facility has recently applied for the Chester County Certified Food Manager (CFM) Certificate. Post CFM Certificate in public view.

Michelangelo’s Italian Restaurant LLC, 1223 Horseshoe Pike, Downingtown, Follow-up, November 9. Pass. No violations.

Motto Japanese Restaurant, 668 Lancaster Ave. Berwyn, November 9. Fail. Observed two (2) opened cans of food inside the walk in refrigerator. Once cans of foods are opened the contents must be transferred to a glass or plastic container. Correct today. Time/Temperature Controlled for Safety (TCS) ready-to-eat food prepared in the food facility and held for more than 24 hours, located in the Sushi Prep Refrigerators; PC cups in Kitchen refrigerators and in the Walk In are not being date marked. Correct today. Repair the slow drains to both hand sinks in the Sushi Area. Correct within 7 days. Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was 0 ppm, and not 50-100 ppm as required. A new container of sanitizer was installed and the chlorine level then found at 100 ppm. Must use test kit everyday to ensure proper concentration. One handwash sink in the Sushi Area being used as a food preparation sink as evidenced by observed packages of salmon resting inside. This was explained on site and corrected. Comply everyday. Food ingredient storage containers, in the kitchen and back storage room, are not labeled with the common name of the food. Correct today. Sushi Prep Area: Observed the sushi rice container stored directly underneath the paper towel dispenser-water from wet hands drips on top of it. Must find another place to store the rice container or move the paper towel dispenser in a central location to be used by both hand sinks. Correct today. Sushi Prep Area: Packages of frozen smashed salmon were observed thawing in a bowl of standing water on the counter; and some were observed thawing in the hand sink and other frozen fish packages were observed on the counter-none of these are approved thawing methods. This was explained and corrected on site. Comply everyday. Utensils hanging from the magnetic strip were observed to have food residue and were not clean to sight and touch. They were taken down to be cleaned and sanitized in the dish washer. Comply everyday. No paper towels were available at the hand sink on the left in the Sushi Prep area. Paper towels were supplied on site. Install a paper dispenser within 7 days. Observed sleeves of take out containers out of their boxes on the shelf in the basement in a room without a ceiling. These items must be moved to a room with a ceiling or be stored in their boxes. Correct today. Replace/repair two (2) light fixtures in the kitchen. Correct within 7 days. Mops are not being hung to air dry. Install mop hangers within 7 days.

Nudy’s Cafe at the Grove, 30 Liberty Blvd. Suite 100, Malvern, November 9. Fail. Dry food ingredient storage containers are not labeled with the common name of the food. Correct today. Observed numerous containers of food stored uncovered in both food prep tables. Foods must be covered. Correct today. Commercially processed ready to eat food, the large bottles of salad dressings, located in the walk in, and held more than 24 hours, are not being marked with the date it was opened. Must contact the manufacturers to determine a discard date as well. Correct today. Time/Temperature Controlled for Safety (TCS) ready-to-eat food prepared in the food facility and held for more than 24 hours, located in the food prep tables, squirt bottles of sauces, pc cups, and in the walk in refrigerator-scrapple- are not being date marked. Correct today and comply everyday. Observed a green cutting board in disrepair in that it was very worn and not cleanable. It was discarded on site. Install broom hangers – the brooms were observed on the floor. Correct within 7 days. Food Employee observed changing tasks that may have contaminated hands without changing their gloves and a proper hand wash in-between. All employees must change gloves, wash hands and don new gloves after changing tasks. This was explained and corrected on site. Re-enforce with all employees today. A squirt bottle of Tahnini Sauce was held at room temperature on the toaster oven- rather than 41°F or below as required. The contents were discarded on site. Chopped chicken was found held at 51°F, and chopped sausage was found held at 43°F in the top of the food prep table, rather than 41°F or below as required. Employee informed that the items were cooked this morning. Food prep tables are not made to cool food; must cool such items in the walk in or large refrigerator. Correct today and comply everyday. Please submit further information on the clear plastic bags that are used to store foods-they must be approved for food contact and heat use. Supply within 7 days. Old unused fryer stored in rear kitchen area, must be removed from food facility. Correct within 7 days. One spatula was observed being used to stir/flip raw eggs on the grill and then immediately used to serve hash browns and cooked meats. This is causing cross contamination. Use on spatula for the cooking of eggs-it should also be switched out with a clean spatula every 4 hours. Use another spatula for handling home fries. Correct immediately and comply everyday. Observed roach bait (a brown peanut butter-like substance) in small dollops along one side of one doorway on the wall on kitchen side and around the electric outlets on the same wall, and along the top of the cove base molding on the same wall. Pest Control Operators report was produced on site but the name of the chemical and the placement of the chemical was not noted. The pest control operator must write complete reports and submit within 7 days.

Reeceville Elementary School, 200 Reeceville Rd. Coatesville, Follow-up, November 9. Pass. The GM stated the dishwasher has been serviced and partially repaired. The technician is still awaiting a part to be received. Repair upon receipt. Maintain all equipment in good repair. De-scale the dishwasher and associated tables and flooring prior to use. Lower steamer is still in disrepair. The GM stated both repairs are on-going, as technicians are awaiting parts to be received. Maintain all equipment in good repair.

Slovak American Club, 310 Gold St. Phoenixville, November 9. Pass. Debris build up in reach in beer cooler under storage rack behind main bar. Clean.

Tira Misu Ristorante, 720 Lancaster Ave. Berwyn, November 9. Pass. Food facility operator failed to post an original, valid Chester County Health Department Certified Food Manager in a location conspicuous to the consumer. Post all licenses in public view as discussed, possible by pantry door area. Floor in kitchen is vinyl and is cracked/roughened/peeled/missing and is not a smooth, easily cleanable surface. Sean indicated a new vinyl floor will be installed in early 2024. Ensure floor cove base is installed or maintained. The light intensity in the food preparation area is not at least 50 foot candles. Replace two light bulbs at light along cook line ceiling light. The following equipment not observed in good repair: Provide updated ansul and fire extinguisher tags dated within one calendar year. The ice chute, lower level, a food contact surface, was observed to have build up and was not clean to sight and touch. Clean and sanitize routinely as needed.

VK Brewing & Eatery, 693 E. Lincoln Highway, Exton, November 9. Fail. Squeeze bottles throughout the facility are not labeled with the common name of the food. Working containers holding food/food ingredients that are removed from their original packages, such as oils, flour, herbs, salt, spices, and sugar shall be identified with the common name of the food. Label all squeeze bottles and dry good containers today. Observed in-use knives stored between table edges or between tables, an area not easily cleanable & sanitized. Relocate knives. Observed foods stored directly on the floor in walk-in cooler, rather than 6 inches off of the floor as required. Move all foods to be 6 inches off of the floor as required. Food facility operator failed to post an original, valid Chester County Health Department Certified Food Manager in a location conspicuous to the consumer. Observed an opened metal can being stored in the 2 door cooler at the cook line. Metal canned foods, once they are initially opened, must have the remaining contents that were not used placed into food grade storage containers. The original packaging must be discarded/recycled, as it is subject to oxidation/rusting once opened. Provide food-grade containers today. Raw eggs stored above ready to eat lunch meats in the Avantco prep top. Eggs were relocated during the inspection. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Clean the soda gun holders at the bar. Observed cheese being prepared in a container stored on an unsanitary surface. Discontinue practice. Time/Temperature Controlled for Safety (TCS) ready-to-eat food prepared in the food facility and held for more than 24 hours, throughout the facility, is not being date marked. Re-train staff. Shellstock tags are not maintained for 90 days from the date the container is emptied. Begin practice immediately. Paper towel dispenser empty at the handwash sink in the kitchen. This is the only handwashing sink in the kitchen and must be supplied at all times. Paper towels were supplied during the inspection. Screen door in kitchen observed in disrepair and areas are not screened. Repair screen. Water damage observed on the ceiling at the bar entrance. Repair/resurface ceiling.

Wood Co./Sodexo @ CFS School at Church Farm, 1001 E. Lincoln Highway, Exton, November 9. Pass. Ceiling tile missing outside the walk-in freezer. Replace ceiling tile. Chocolate milk, house made chipotle mayo, and house made cilantro mayo, potentially hazardous ready to eat foods requiring datemarking, in the walk-in cooler, were beyond the date-marking and required discarding. Chocolate milk dated 10/27 was discarded. House made cilantro mayo dated 10/20 was discarded. House made chipotle mayo dated 10/20 was discarded. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Clean the following: Ice machine vent cover; Interior dining center freezer chest; Floors under the mechanical dishwasher and 3 compartment sink; Floors and back/sides of equipment at the cookline.

Zurita’s Tacos/CCHD #1290, 915 Penn Green Rd. Avondale, November 9. Fail. Foods were sitting on counter and stove shelf. No temperature or time tracking was being done to ensure proper cooling of foods. A food temperature log was left at the lunchera. Must take temperature of each food and write the time when foods that are cooling reach 135°F. When foods reach 135°F, 2-hour window to get food to 70°F then 4 more hours to get food to 41°F. When foods reach 90-100°F, place them in the refrigerator (uncovered) so they can cool quickly.  Numerous flies were in the lonchera. While serving window and doors are properly screened and were closed when inspection began, a lighted fly trap is needed. An example of a proper trap with a glue board was shown to the operator. Purchase today and install when it arrives. Flies must be eliminated. Remove the old refrigerator that is outside. Grocery bags used to line tortilla maker. These are not food grade bags. Use “deli paper”, waxed paper or parchment paper – food grade paper.

1339 BBQ LLC @ The Brickette Lounge, 1339 Pottstown Pike, West Chester, November 8. Pass. Food ingredient storage containers (dry goods in the basement) were not labeled with the common name of the food. Containers were labeled during the inspection. Food dispensing utensil in downstairs dry goods observed stored in the food and not with handle above the top of the food. Plastic bowls being used as scoops were removed during the inspection. Replace with scoops and always store with the handle above the food. House made coleslaw and house made potato salad dressing, potentially hazardous ready to eat foods requiring datemarking, in the 2 door cooler by the cook line, was beyond the date-marking and requires discarding. Soda gun nozzle at the bar was observed to have residue and were not clean to sight and touch. Nozzle was cleaned and sanitized during the inspection. Clean the following: legs of the 3 compartment sink at the bar next to the dishwasher; hood filters at the cook line; floors under the dishwasher in the kitchen; a/c unit next to the dishwasher in kitchen. Observed a leak at the food prep sink. Repair leak and send proof of correction to the Department.

Open mug of coffee observed on cutting board in kitchen prep area, indicating staff is drinking while working. Staff may not eat or drink while working. Store personal food away from facility food and equipment. Staff beverages must be in lidded containers. Chef observed slicing two croissants with bare hand. Bare hand contact with ready to eat foods is prohibited. Use gloves or utensils in order to prevent bare hand contact with ready to eat foods. Wiping cloths used on food contact surfaces (such as cutting boards & prep table along mainline) must be stored in a sanitizer solution. Facility is reusing single-use containers for food storage. Discontinue this practice and use food grade containers. Plastic “take-out” style bags may not be used to store unpackaged foods. Use food grade bags for storage of unpackaged foods. Remove all cardboard lines from floors and shelves as absorbent materials are prohibited to be used as liners. Remove all milk crates from the facility that are in use as shelves and storage bins. Crates may only be used for transport of items (via delivery etc.). Provide stem-type thermometers for taking internal temperatures of cold foods. Note: Probe thermometer for hot foods is available.

Philly Pretzel Factory/Uwchlan, 126 Eagleview Blvd. Exton, Follow-up, November 8. Pass. Numerous dough platforms exhibited black discoloration. Have the platforms properly cleaned, refinished or replaced. Food contact surfaces shall be maintained in a clean and sanitary condition, clean to the sight and touch. Staff noted the owner will be sanding the platforms. This is a repeat violation. Failure to correct will result in non-traffic citations and follow-up fees being issued against you. Provide the Department with the Philadelphia Pretzel Factory standard operating procedures for cleaning dough platforms. The following structure was observed in disrepair: Walk-in flooring. An attempt at patching the vinyl floor has been completed. The patch has rough edges that are chipping up and breaking. By December 15, a new smooth, durable, easily cleanable must be installed in the unit. Previous to install, the flooring must be Department approved. The wooden cove base against the half wall in the kitchen area is improper. A bottom is not tightly sealed allow for water liquid transfer and soiled accumulations. Replace with a tight fitting vinyl or tile cove base. Correct by Dec. 15, 2023. The existing wood cove base is not sufficient liquids from leaching into the wallboard. Correct all within 45 days.

Ye Old Towne Deli, 302 Main St. Parkesburg, Follow-up, November 8. Pass. The floor in the back prep room and the floor in the dishwashing room were observed to be damaged. Facility in the process of getting repair quotes and requested more time to repair and plan for temporary closure for repair. Correct within 60 days. Wall in dishwashing room next to ice machine is damaged. It was reported that wall will be repaired when floor is repaired. Facility is in the process of getting repair quotes and requested more time. Repair within 60 days.

Olive And Meadow, LLC, 208 Carter Dr. Unit 13b, West Chester, November 6. Pass. No violations.

Urbn Devon Yard LLC, 138 Lancaster Ave. Suite 140, Devon, November 6. Fail. In-use sauce bottles and/or oil bottles located in the kitchens of the Terrain Cafe, Amis kitchen, and the special events kitchen were observed without labels. Ensure all bottles are labeled with the common name of the food. Observed in-use knives being stored between countertops (a not easily accessible or cleanable area) in the front area of Pizzeria Vetri. Knives were removed and washed. Ensure knives are stored in a designated area that can be easily cleaned to avoid contamination. Observed single-use plastic containers being used as a food dispensing utensil in the shared prep area of Pizzeria Vetri and Amis. Cups were replaced with scoops with handles as required. Roasted carrots in the prep top cooler of Amis kitchen were being held at 53°F as opposed to 41°F or below as required. The carrots were above the fill line in the storage container and discarded by Person in Charge. Ensure food is below fill lines in containers in cold holding units to ensure food remains at required temperatures to avoid pathogen growth. Walk-in refrigerator in Terrain cafe area observed at 42°F (ambient air, caesar dressing, vinaigrette temped). Person in Charge adjusted temperature and temperature was reduced to 39°F. Monitor temperature in walk-in refrigerator to ensure a temperature of 41°F or below is maintained. The following were not date-marked: Deli meats in the walk-in cooler for Amis. Discarded. Prosciutto and marinara sauce in the walk-in cooler for Pizzaria Vetri. Date-mark for day they were sliced or made applied. Ensure all ready to eat foods are date-marked with the day they were prepared and discarded after 7 days to avoid the growth of harmful pathogens. In Terrain Cafe kitchen, the 3-compartment sink did not have any available test strips. PIC got extra test strips from the bar area. Ensure test strips are available at all times for reading the proper concentration of sanitizer solution (200-400ppm). In the Terrain Cafe kitchen. Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was 25 ppm, and not 50-100 ppm as required. Chlorine sanitizer bucket was replaced and chlorine sanitizer residual read 50ppm as required. Ensure to use test strips and replace sanitizer solution as needed. In the Terrain Cafe and Pizzeria Vetri kitchens the ice machines were observed to have a bio-film growth. The ice must be emptied and the interior of the ice chutes cleaned before use. Clean these food contact surfaces routinely avoid any growth of potential pathogens. In special events kitchen area. Observed a buildup of grease and residue on the hood vents above grill. Clean immediately. The hand sink in the front area of Pizzeria Vetri was blocked by a food processor. Ensure the hand sinks are empty, unblocked, and easily accessible at all times. Paper towels were not available at the front hand sink and espresso area hand sink for Pizzeria Vetri. Ensure paper towels are supplied at all times to hand sinks. A fly trap and bottle of cleaner was observed on the drying rack for the clean dishes in the Terrain Cafe kitchen. Fly trap and cleaner were removed. Ensure all chemicals or toxic substances are in a designated area away from food or food contact surfaces.
