LANSING, Mich. (WILX) – People will be able to voice their concerns regarding health in the community.

Health departments across Mid-Michigan are launching a community health needs assessment called Healthy! Capital Counties project. The goal is to identify the most pressing health concerns within Clinton, Ingham and Eaton counties.

The survey is available from April 15 to May 31. People can take the survey online or complete a paper survey at the following locations:

• BEDHD Charlotte Office at 1033 Healthcare Dr. Charlotte, MI 48813
• ICHD Office at 5303 S Cedar St. Lansing, MI 48911 (Health & Resource Navigation Area)
• MMDHD Office at 1307 E. Townsend Rd. St. Johns, MI 48879
