
SYRACUSE, N.Y. (WSYR-TV) — The Onondaga County Health Department released its inspection reports for restaurants checked from March 17 through March 23.
Two food services failed their inspection:
- King Chef – 911 Old Liverpool Road, Liverpool
- Koto Japanese Steak House – 9331 Destiny USA Drive, Syracuse

King Chef
King Chef had 14 violations, one in critical condition.
Critical violation: Improper cooling and refrigerated storage of potentially hazardous foods
The inspector found one dozen cooked chicken wings on the cookline at 68-75 degrees Fahrenheit. The wings were in a plastic food pan and according to the female employee, they were cooked and removed approximately 45 minutes ago and placed there to cool in room-temperature storage. This was corrected and the wings were put into the walk-in cooler to finish the cooling process.
Food not protected in general: The inspector noted that most bulk-bagged products weren’t stored closed and left open.
Food not protected in general: The inspector noted that the single service items used in bulk products lacked handles. The single service items also weren’t stored inverted as required.
This violation was found two times.
Poor sanitary design, construction, and installation of equipment and utensils: The inspector noted that the racks in the walk-in cooler were rusty and several lower shelves of tables were covered with foil, which is not a smooth and easily cleanable surface.
This violation was found two times.
Improper cleaning, washing, and sanitizing of equipment and utensils: The inspector noted that several wet wiping cloths were not stored in sanitizer following use.
Improper cleaning, washing, and sanitizing of equipment and utensils: The inspector noted that the exterior of two rice cookers wasn’t clean and had greasy food soil. The exterior of the microwave oven also wasn’t clean and had greasy food soil. All wooden shelving was not clean as well.
This violation was found three times.
Improper sanitary facilities and controls: The inspector found food soil scraps in the hand wash sink which wasn’t clean.
Improper construction and maintenance of physical facilities: The inspector noted that some floor areas were in poor repair with missing tiles. Most floor areas under and behind equipment weren’t clean as well.
This violation was found two times.
Improper construction and maintenance of physical facilities: The inspector noted that the vent hood filters and adjacent areas had accumulated grease and an appointment for cleaning was made in April.
Koto Japanese Steak House
Koto Japanese Steak House had 20 violations, five in critical condition.
Critical violation: Foods adulterated or received from unapproved sources
The inspector noted the vacuum breaker on the mop sink faucet was in poor repair.
Critical violation: Foods adulterated or received from unapproved sources
The inspector found specks of mold on ice in the ice machine. This was corrected and the ice was voluntarily discarded.
Critical violation: Foods not protected from contamination, temperatures not measured
The inspector found containers of raw chicken and other raw meat products were stored in preparation on top of the cooler at the cook line, directly next to ready-to-eat food and garnishes including cherries and kale. This was corrected and the raw products were rearranged and moved to prevent cross-contamination.
Critical violation: Foods not protected from contamination by other sources
The inspector found containers of rice seasoning stored on a shelf in the hallway kitchen area directly next to toxic chemicals. This was corrected and the rice seasoning was moved from shelves. The inspector also found a squeeze bottle of hand soap at the takeout window not labeled and stored in a reused/recycled plastic food bottle. This was corrected and the bottle was discarded while a new bottle of labeled soap was provided.
Critical violation: Improper cooling and refrigerated storage of potentially hazardous foods
The inspector found one large bulk container with thirty gallons of beef-based teriyaki sauce at 68 degrees Fahrenheit stored in the walk-in cooler. According to the kitchen manager, the sauce was prepared and cooked the day before and left in the kitchen to cool for a while before placing it in the walk-in cooler. This was corrected as the product was not properly cooled, and 30 gallons of the sauce was voluntarily discarded.
Food not protected in general: The inspector noted that various squeeze bottles of sauces and condiments throughout the kitchen weren’t labeled. Containers of food were also stored on the floor in the walk-in cooler, walk-in freezer and in the back dry storage area. Various food items were also double-stacked in refrigeration units throughout the kitchen.
This violation was found three times.
Food not protected in general: The inspector noted that the knives were improperly stored between equipment, which is not a clean and sanitary area. This was corrected. The inspector also noted scoops stored in bulk food products with handles touching the product.
This violation was found two times.
Food not protected in general: The inspector noted that some refrigeration units lacked thermometers.
Improper cleaning, washing, and sanitizing of equipment and utensils: The inspector found wiping cloths not stored in sanitizing solution between use.
Improper cleaning, washing, and sanitizing of equipment and utensils: The inspector noted that the interiors of some coolers in the kitchen weren’t clean, including racks and door gaskets. The interior of the ice machine was clean too.
This violation was found two times.
Improper cleaning, washing, and sanitizing of equipment and utensils: The inspector noted that shelving throughout the kitchen wasn’t clean.
Improper sanitary facilities and controls: The inspector noted that the handwash sink near the dishwasher in the kitchen was obstructed by items stored in the sink. Two hand sinks in the kitchen lacked paper towels. This was corrected.
Improper construction and maintenance of physical facilities: The inspector noted that floors under and behind equipment throughout the kitchen weren’t clean. Some wall areas in the back hallway area were not clean and floor tiles at the front takeout area near the grill were in poor repair.
This violation was found three times.
Improper construction and maintenance of physical facilities: The inspector noted that the light bulb in the walk-in cooler wasn’t shielded and ceiling vents in the hallway area weren’t clean.
Many locations passed their inspections between March 17 through March 23.
